Confidentiality and Use Agreement for Grant Review Process
The Glut1 Deficiency Foundation Scientific Advisory Board is responsible for reviewing, evaluating and reporting on grant applications and proposals for the purpose of advising the Glut1 Deficiency Foundation Board of Directors in connection with grant award allocations. Members of both the Scientific Advisory Board and the Board of Directors who receive grant applications and proposals for review must:
- notify the Glut1 Deficiency Foundation if any conflicts of interest exist between any proposal or applicant and the recipient, including financial interests and personal relationships that could result in bias;
- hold all grant applications and proposals in the strictest confidence, and not disclose, in any manner, information about or contained in the applications, proposals or any related discussions, evaluations, or documents to anyone who has not been designated by the Glut 1 Deficiency Foundation to participate in the grant review process;
- not use information about or contained in any grant application or proposal for any purpose other than the purpose described above; and
- at any time at the request of the Glut 1 Deficiency Foundation, return to the Foundation or destroy all grant applications and related materials.
