Dr. Juan Pascual Clinic Update

Announcement shared on behalf of Dr. Juan Pascual:

Dr. Pascual’s G1D clinic at Weill Cornell Medicine will open in mid-March. The telephone number for appointments and inquiries is (212) 746-3278. No video consultations are offered. More information is available at: https://weillcornell.org/juan-pascual-md

Two legal requirements apply to medical consultations across the U.S. now:

1) A written physician referral is necessary because state laws prohibit the prescription of medications or tests for patients when they return to their state of residence. The referral ensures: a) agreement to continuity of care by the patient home institution and b) the written transmission of any recommendations for evaluation by referring physicians.

2) Electronic medical records from one institution (for example, Children’s Health Dallas or UT Southwestern) are not automatically accessible from other institutions (including Weill Cornell Medicine). However, when the records exist in EPIC, electronic access can be granted by patients to all other medical institutions that use EPIC (including Weill Cornell Medicine) via direct patient request to the institution that holds their medical record. Medical institutions (including Weill Cornell Medicine) cannot request or convey requests for this access. Only patients can. When EPIC is not available, paper records are acceptable.

Useful medical records authorization information:
The number for Children’s Health Dallas (pediatric patients) is 214-456-2509 and the number for UT Southwestern (adult patients) is 214-645-3030.

Patients can request that their EPIC medical records be made accessible through “EPIC Care Everywhere”. This enables EPIC access by any institution that the patient may visit anywhere including, but not limited to, Weill Cornell Medicine.
